Is it really possible for implants to be applied immediately after extraction, during the same procedure?
It sure is!
More than ten years ago, many international studies on tens of thousands of patients have shown that this technique is absolutely effective and the implants have the same success rates as implants applied after the extractions were performed and the area was left to heal for a period of about 40 days after surgery.
This technique was born from the need not to leave patients without teeth for such a long period of time and to avoid wearing uncomfortable mobile dentures, which are not at all aesthetically appealing.
What are the additional advantages of this formidable technique, since it works exactly like the traditional one in terms of results:
1 A single surgery, a single anaesthesia, a single antibiotic treatment and a single day of discomfort
2 A better aesthetic result because immediate implant placement reduces bone and gum resorption and thus the teeth look more natural and correctly sized
3 Decreasing the need for bone grafts (and therefore lower costs) due to the lower resorption obtained by this technique
4 Possibility to have a fixed denture in 48 hours, eliminating the discomfort of a possible mobile denture in the post-extraction period, if implants are to be placed later
Then why isn’t this technique used and promoted by all dentists?
Well, for two reasons:
- Lack of scientific knowledge among dentists, who are not aware of international studies and consequently do not feel confident and do not trust the procedure, preferring to wait, thus reducing the aesthetic performance of the final procedure and increasing discomfort for the patient all this time
- The greater technical difficulty in performing the procedure, which is much more technically complex, so that dentists who do not have sufficient experience (and that means those who place less than 3-400 implants per year) are afraid to do it; increased difficulty is not negligible as the implant should be placed in the hole left by the tooth root, which is sometimes very thick and leaves little room for screwing the implant.

Obviously, this technique can be used after a careful clinical and radiographic analysis and it is always necessary to perform a CT of the affected area (at our clinic, this can be done during the same session because we have the equipment directly right here) to carefully examine both the shape of the roots and the thickness and height of the available bone.
